In England, ENT is one of the most congested specialties in the public system. More than half of patients exceed the 18-week target between GP referral and treatment. The American Heart Association recognizes it as an independent risk factor for stroke and recurrence, and a landmark New England Journal of Medicine study had already shown an excess risk of stroke and death when apnea is left untreated.

On treatment, let us be precise. Continuous positive airway pressure improves symptoms, alertness and quality of life, and its cardiovascular benefit is mainly demonstrated in severe forms and in patients who use it regularly. The point is to diagnose early and treat the forms that warrant it.

The other ENT reasons that should not drag on

Chronic nasal obstruction and allergy, allergen immunotherapy is the only treatment that modifies the natural history of allergic disease and, in selected patients over at least three years, can reduce progression to asthma. Throat, tonsils, voice and swallowing, persistent symptoms deserve an examination. Vertigo and tinnitus can point to a treatable cause. Some situations are emergencies, sudden hearing loss, facial palsy, a persistent neck mass, and for any sudden neurological sign the right reflex is to call emergency services first.
